  Surgical Alternatives to Facelifts

If a standard (or traditional) facelift or a mini-facelift for the middle portion of the face is not a good option for a patient, there are other facial plastic surgery procedures to consider. Possible alternative procedures include the following:


  Brow (or Forehead) Lift

Ideal for the lines and furrows on the forehead, your doctor can perform a brow lift to correct a sagging or deeply furrowed brow.

Brow lifts can be performed in different ways. Your doctor may perform the operation using an endoscope (a thin tube with a camera on the end) and special instruments placed through small incisions made within the hairline. This allows the tissue and muscle beneath the skin to be adjusted, correcting the source of visible creases and furrows in the forehead.

Your doctor may also perform a brow lift through a "coronal incision" The coronal incision is created from ear to ear across the top of the head within the hair-bearing skin.

The forehead skin is then lifted and any muscle adjustment can be made under direct visualization. The resulting scar from a coronal brow lift is well concealed within the hair. For patients with very high foreheads, an incision just at the anterior hairline may be recommended. In this way the brow can be lifted without altering the height of the front hairline. The resulting scar, while somewhat more visible, can often be hidden with bangs.


  Neck Lift

Some people feel they are not ready for a full facelift because their upper face is still pleasing. However, many patients will note excess wrinkling of the neck skin, a double chin or "turkey wattle" and jowl lines. The loss of youthful contours in the face and neck can be due to a variety of factors, including heredity, gravity, environmental conditions and stress. When the neck area doesn't match the upper facial appearance, a neck lift may be a good solution.

Ideal for loose and flabby skin of the neckline, a necklift is a surgical procedure that improves visible signs of aging in the jawline and neck, and addresses the sagging jowls, loose neck skin and fat accumulation under the chin. This technique reduces the excess skin and banded appearance of the neck area, and tightens the neck area by tightening, realigning, or reducing neck muscles, producing a more youthful appearing neck with a well-defined contour. The neck lift incision often begins in front of the earlobe and wraps around behind the ear, and ends in the posterior hair behind the ear.

What neck lift surgery can treat:

  • Excess fat and skin relaxation in the lower face that creates jowls
  • Excess fatty deposits under the chin
  • Loose neck skin
  • Muscle banding in the neck, which created abnormal contours

  Facial Fat Grafting

A youthful look depends on having the right amount of facial fat in the right locations. As we age, redistribution, accumulation, and atrophy of fat deposits can lead to facial volume loss. As some areas lose fat, such as the forehead and cheeks, and other areas gain fat, such as the mouth and jaw, the contour of the face changes for the worse. Additionally, areas of fat become farther apart, and instead of a smooth, almost continuous layer, the fat pads appear as separate structures.

Facial fat grafting is used to restore lost facial volume, by using your own fat, which is collected by gentle liposuction from an area of excess fat on your body, then carefully prepared and injected (“grafted”) into areas of deficiency. The results from this may be permanent, and this may be appealing for patients who want a more long-lasting outcome than can be achieved from soft-tissue fillers. When used in conjunction with other surgical facial rejuvenation surgery, the results can be dramatically rejuvenating.

Facial fat grafting is often done as a complimentary procedure during facial surgery, to give a more dramatic rejuvenation outcome, although may be also done as a stand-alone procedure if suitable.


  Full Face Lift

The most comprehensive facial rejuvenation procedure addresses all areas of the face, from the forehead all the way down to the neck. It is used for patients who have signs of advanced aging. This will provide the most dramatic results, as well as the most long-lasting outcome for the patient.

A full facelift generally positions the incision along the hairline, extending down to the temples, and stopping at the base of the ears. The underlying facial tissues and muscles are pulled and tightened to create a lifted appearance, which reduces the excessive wrinkles and sagging. The full procedure may include a brow lift, face lift, neck lift, eyelid lift to rejuvenate aging eyes, and facial fat grafting to add fullness to the lips and cheeks and reduce the appearance of wrinkles, though only the components that you require and desire will be recommended.
